We are hiring for a Senior Support Coordinator for the Health Centre in our Mastrs & Oasis Program and are looking for you. You will be an integral part of a collaborative, inter-disciplinary team focused on delivering seamless programming and service excellence to our patients.Hours of work: Temporary Full Time, (up to 6 months)The overarching goal of the Senior Support Coordinator is to provide appropriate, empathetic, non-judgmental and respectful care for older adults with serious mental illness and concurrent disorders while ultimately helping the client to increase their quality of life and decrease the time spent in hospital. If you are looking for a company that is constantly moving forward in pursuit of successful outcomes, get ready to love it here!What you'll be doing: * Provide evidenced-based support coordination for seniors with serious mental health and concurrent disorders * Conduct comprehensive patient assessments * Provide support coordination within a recovery framework, inclusive of treatment and connecting to community supports by engaging both formal and informal networks * Ensure clients are supported in community environments, with 75% of services provided in community locationsWhat we're looking for: * Bachelors in Social Work or other related field * Valid registration with the relevant college/governing body in Ontario, such as the Ontario College of registered Psychotherapists (CRPO), or in pursuit of relevant credential is mandatory * Knowledge of older adult population needs and concurrent mental health disorders * 1 - 3 years' experience in concurrent mental health and knowledge of the complexities within the senior population * Valid driver's license, business insurance and personal vehicleAs part of our commitment to the health and safety of our patients, staff and community from COVID-19, subject to any accommodation required by applicable human rights legislation, Homewood Health Centre requires that all staff have received all required doses of a COVID-19 vaccine approved by Health Canada.Why Work For Us Homewood Health is Canada's largest and leading service provider for medically focused, evidence-based treatment of mental health and addiction disorders.
